Unless he has eight clubs, Declarer will need the heart king to have any chance.  If he has three hearts, then the suit will run for twelve tricks; also, he would have raised hearts at some point.  If he has a 7-card club suit, then there are twelve tricks on top, and declarer would instead be trying for an overtrick.  Therefore, we place him with a 3-2-2-6 pattern, and his hand should look like this:
